THIS DEPOSIT WILL BE USED TO COVER ANY DAMAGES, OR CLEAN UP, TO COMMON PROPERTY, RESULTING FROM YOUR CONTRACTOR(S) PERFORMING THE WORK ASSOCIATED WITH THIS REQUEST. Any resident wishing to make any addition or alteration to the exterior must complete and submit this form to the Management Company. No work may be commenced until an approval has been provided, as per the Gramercy Park by-laws and Covenant. Prior to commencement of the work, a copy of the appropriate building and zoning permit must be submitted to the Management Company for the Association’s records.
Approval is required for the following alterations. Please specify the proposed alteration, including color, type of materials, style/shape: explain in detail the proposed alteration. Include a photograph of the property (entire view). Attach a copy of the site survey with proposed modifications, showing specifications for all proposed alterations.*(Required)

Site or piping plan (Original plans signed by Architect are required for architectural changes). 4. Survey (Pool, Driveway, and any other architectural change) 5. Required Permits (Permit Card and Notice of Commencement from Town of Miami Lakes) 6.
